無托槽隱形矯治器磨牙遠移時前牙的應力分析

摘要:目的:利用有限元法分析無托槽隱形矯治器推磨牙遠移時,施加不同支抗力值后前牙區的生物力學效應,探討磨牙遠移的支抗需求。方法:建立上頜牙列-牙周膜-牙槽骨-無托槽隱形矯治器模型,依據磨牙遠移的形式分為Ⅰ組(單獨推一顆磨牙遠移)、Ⅱ組(同時推兩顆磨牙遠移),依據在尖牙處施加支抗力值的大小分為:ⅠA(0g)、ⅠB(100g)、ⅠC(150g)、ⅠD(200g);ⅡA(0g)、ⅡB(100g)、ⅡC(200g)、ⅡD(300g),在有限元分析軟件中對比分析前牙區的牙周膜應力情況及位移趨勢。結果:Ⅰ組使用無托槽隱形矯治器單獨推一顆磨牙遠移時,在尖牙處施加100g支抗力值,能有效地減小前牙唇向位移,同時牙周膜應力最??;Ⅱ組同時推兩顆磨牙遠移時,在尖牙處施加300g支抗力值,能有效地減小前牙唇向位移,同時牙周膜應力最小。結論:選擇不同的磨牙遠移模式,可增加相應力值的支抗,當選擇推一顆磨牙遠移的"A"模式移動時,可施加100g支抗力,當選擇同時推兩顆磨牙遠移的"V"模式移動時,可施加300g支抗力。

The stress analysis of the anterior teeth of the molar distalization with clear aligner
Abstract:Objective: Use the finite element method to analyze the biomechanical effects of the anterior teeth during the process of molar distalization by applying different anchorage values and discuss the needs of anchorage for molar distalization. Methods: A three-dimensional finite element model of the maxillary denture- periodontal membrane- alveolar bone-clear aligner was established. According to the different forms of the molar distalization, it is divided into groupⅠ (second molar moved only) and groupⅡ (first molar and second molar moved at the same time). According to the value of the anchorage applied to the canine, the group is divided into:ⅠA (0g),ⅠB (100g),ⅠC (150g), andⅠD (200g);ⅡA (0g),ⅡB (100g),ⅡC (200g),ⅡD (300g). The stress of the periodontal ligament and the displacement trend of the anterior tooth are compared and analyzed in the finite element analysis software. Results: When groupⅠusing clear aligner to move a molar far away, applying 100g anchorage to the canine can effectively reduce the anterior teeth labial displacement, while the periodontal ligament stress is the smallest. When groupⅡusing clear aligner to move two molars far away, applying 300g anchorage to the canine can effectively reduce the anterior teeth labial displacement, while the periodontal ligament stress is the smallest. Conclusion: Choose different forms of molar distalization, the anchorage of the corresponding force value can be increased. When the "A" mode to push one molar away was chosen, 100g anchorage force can be applied. When the "V" mode to push two molars away at the same time was chosen, 300g anchorage force can be applied.
